You may use our Referral Request form below to request child care referral information for Washtenaw, Hillsdale, Jackson, Lenawee and Monroe counties. Please fill out the form, completing all relevant information. If more than one choice applies to a particular question, please select all that apply to your situation.

We will compile a list of providers who match your requirements. Referral requests will be completed within 2 business days. The information will be sent to you via mail, fax, or e-mail, depending on which option you select.

If you are looking for child care outside of our five county region, please visit the Michigan 4C agency for addition offices in Michigan or Child Care Aware for offices across the United States.

CCN does not share client information with outside agencies or businesses, nor sell its client information to outside parties. We share information only if required by our contracts with funding sources such as Michigan DHS.
